A. Verifivation of Age and Capacity *

I verify that I am of legal age of majority (>18 years) at the time that I am participating in this photography shoot. I further verify that I have full legal capacity to execute this authorization without the consent or knowledge of any other person. I understand that I will be required to present proof of identity and age at the time of the photographic session.

B. Photographic Release *

I understand that I will be providing makeup / styling services for a model who is participating in a photographic shoot and that my work will be photographed. I hereby grant the undersigned Photographer permission to photograph my work. I further give my irrevocable consent to the Photographer and his direct or indirect licensees and assignees to publish, republish, reproduce, distribute, or otherwise transmit the images of my work in any medium for all purposes, commercial or non-commercial, throughout the world. I understand that the images may be altered or modified in any manner. I hereby waive any right that I may have to inspect and approve a finished product or the copy that may be used in connection with an image that the Photographer has taken of my work, or the use to which it may be applied. I further release the Photographer and his direct or indirect licensees and assignees, from any claims for remuneration associated with any form of damage, foreseen or unforeseen, associated with the use of these images.

C. Terms for Time for Images (TFI) Photo Session *

I verify that I have entered into a Time-for-Images (“TFI”) agreement with the photographer. I understand that this means that in addition to my financial compensation for my time, my travel, or for any other forms of professional services that I may render as a part of this photographic session. I understand that the agreed upon number of images will be made available electronically over the internet and that it will be my sole responsibility to access and to download these images. I understand that limited copyright authorization has been extended to me only for the specific pictures that the photographer selects and that I subsequently will receive through this agreement and that full copyright for any other pictures taken during this session is retained by the Photographer. The limited copyright extension for the pictures that I have received through this TFI agreement allows me to use said pictures for my personal portfolio use only. I understand that full commercial copyright for these images is retained by the photographer and that I or my agent may not use these images for commercial purposes without the expressed, written consent of the photographer.

D. Financial Compesation *

In exchange for providing hair and make-up services in a manner deemed acceptable by the Photographer, I will be financially compensated in the agreed upon amount per photography client.

E. 18 USC 2257 Compliance Statement *

I understand that the photographs taken as a part of this session may contain material that is considered in some locations and/or by some jurisdictions to be adult in nature and/or to contain adult content. I verify that I am of legal age to consent for participation in producing, contributing to, and/or styling of such images, and I understand that there are potentially serious legal consequences to providing false statements or documentation of my age. I understand that copyright for these images is retained by the Photographer and that these photographs may be used for personal, portfolio, and/or commercial applications (including sale and/or distribution). I hereby release all claims against the Photographer and his direct or indirect licensees and assignees for any claims for remuneration associated with production, possession, sale, distribution, or any other use of such photographs.

F. Photography / Video *

I understand that a combination of still or real-time, audio, image, and /or video recording may be in use during this photographic session. This is primarily intended for safety and security purposes, although the use of such recording is not necessarily limited thereto. I hereby agree to be recorded by such devices in said modalities during the entirety of this session. Furthermore, I waive any right that I may have to such recordings and relinquish their ownership and full copyright to the photographer. I further release the Photographer and his direct or indirect licensees and assignees from any claims for remuneration associated with any form of damage, foreseen or unforeseen, associated with the use of these recordings.

G. Timeliness *

I understand that it is my obligation to arrive at the agreed upon location by the agreed upon time and to participate in the photo shoot for the agreed upon period of time. I understand that violating any of these terms invalidates my claim to any form of monetary compensation for this shoot and/or any photographs that I was previously entitled to receive. I further understand that violation of these terms may result in additional financial costs to me associated with lost investments of the Photographer’s time and/or resources and that I may be held legally liable for these charges.
